Elastic venous compression orthosis
Technical area
The present invention relates to a venous compression brace and / or elastic venous compression, or CVE, indicated in case of venous disease of a lower limb of a patient.
State of the art
Venous compression and / or elastic venous compression orthoses, formerly known as "compression socks" or "compression tights", are textile medical devices that provide a therapeutic effect by compression and / or compression. lower limbs, as opposed to "holding stockings" (or "low support" or "low anti-fatigue") and "low mode", which are not medical devices for therapeutic purposes.
Orthotics are designed to produce a therapeutic effect by compression and / or compression of the lower limb over a greater or lesser extent, usually with a decreasing pressure profile upwardly from the ankle.
To allow strong compression of the lower limbs, the elastic venous compression orthoses are made from a knitted mesh incorporating an elastic weft yarn, usually a gimped elastane. The orthosis is threaded, to a service position, on the lower limb of the patient to be treated. The restoring force of the elastic fibers then exerts compression.
The mesh and the yarns, as well as the sizing of the stitch rows, are chosen so as to apply predetermined pressures at different altitudes of the lower limb, for example at the height of the ankle, starting from the calf, at the level of the calf, in the popliteal hollow, etc. up to the upper thigh, altitudes conventionally noted B to G. These different pressures are defined for each class with reference to metrological templates such as the leg-model of the French standard NF G 30-102 part B, appendix B, corresponding to the leg-model "Hohenstein type" according to the German standard RAL-GZ 387, or as defined in the European experimental standard XP ENV 12718: 2001.
The morphology of the lower limbs being different from one patient to another, a model of orthosis is classically declined in several sizes in order to satisfy the target population.
A size of a model is classically characterized by particular dimensions. The elasticity of the orthosis, however, allows a size to suit patients with lower limbs of different dimensions. The set of sizes of a model is called "grid" of sizes.
To reduce costs, orthotics manufacturers need to reduce the number of rack sizes.
They also need to be able to manufacture orthotics with knitting looms that are as inexpensive as possible, and in particular that do not have specific reeling systems such as the Memminger ELAN systems described on the site http://www.memminger-iro.com/en /fournisseure/elan-30.php thislD=151.
Finally, there is a constant need to improve the efficiency and comfort of orthoses.
An object of the invention is to meet, at least partially, these needs. Summary of the invention
According to the invention, this goal is achieved by means of an orthosis comprising an edge-side and a leg portion, the edge-side and the leg portion comprising a knit yarn, or "mesh yarn" or "yarn" "bottom-side" and "leg portion", respectively, defining a network of knitted stitches, and an edge-side and leg-part weft yarn, respectively, each weft yarn having a center yarn, or " soul ", preferably a single core, gimped, preferably double-gimped by means of a cover wire.
Remarkably, the core of the edge-side weft yarn and the edge-side weft yarn cover yarn (s) are elastane and the edge-side weft yarn core has a lower titer than 0.7 times that of the soul of the leg part warp thread.
The inventors have found that an orthosis according to the invention can adapt to a large fraction of the population, which makes it possible to reduce the number of sizes. In addition, surprisingly, without being able to explain it theoretically, the inventors have discovered that the orthosis can easily be manufactured with standard knitting machines. Investments and operating costs are advantageously limited. Finally, it is effective, and particularly comfortable.
An orthosis according to the invention may also comprise one or more of the following optional and preferred features: the edge-side knit yarn is a single gimped yarn consisting of an elastane core covered with a polyamide cover yarn; the ratio of the web title of the leg portion web to the web title of the edge-side weft yarn is greater than 2.0; the web title of the leg portion knit yarn is less than 45 dTex and greater than 10 dTex; the edge-side comprising a reinforcing thread, different from the edge-side weft yarn and the edge-dimension knitting yarn, extending transversely and having an elongation at break, measured according to DIN 53834, lower than 40%; the reinforcing yarn is made of polyamide and has a titre greater than 30 dTex and less than 60 dTex; the edge-side has an inner lapel and an outer lapel, folded over the inner lapel, the reinforcing thread extending exclusively into the inner cuff; the orthosis is an elastic venous compression orthosis of class I, II, III or IV, according to the ASQUAL reference system.

In Figures 1 and 2, reference numeral 10 generally designates an orthosis according to the invention. The orthosis 10, of anatomical general shape, comprises a foot portion 12 wrapping the foot, a leg portion 13 having an ankle portion 14 wrapping the ankle and a calf portion 16 surrounding the calf, and a terminal knitted portion "edge-dimensions" 18. The orthosis 10 extends to a level below the knee, in the case where the orthosis is a sock "mid-low" (or "low-hock").
The configuration in the form of a sock is not limiting, and the orthosis 10 can also be made in the form of a "lower thigh", extended by a portion of the thigh 20 compressive. The orthosis 10 may also be made in the form of a pantyhose, and / or without a foot part 12 (low or "open foot" type).
The various adjoining parts of the orthosis 10 are preferably knitted continuously on a circular loom, according to conventional methods. The realization of the orthosis 10 requires no assembly step for assembling the various parts, except the sewing operations of the tip at the foot portion 12, if the latter is present.
In FIG. 2, the altitudes of the lower limb as defined by the morphological reference indicated in the introduction (leg-model or "template" of the "Hohenstein" type) have been represented, using the standardized notation: - B: peg, at point of its minimum circumference; - B1: junction point between the Achilles tendon and the calf muscles; - C: calf, at the point of its maximum circumference; - D: just below the tibial tuberosity (that is, just below the knee); - E: in the center of the patella and above the back of the knee (that is to say at the level of the popliteal fossa); - F: in the middle of the thigh; and - G: at the top of the thigh.
The calf is the limb segment between levels B1 and D, and the ankle is the limb segment below level B1.
As shown in FIG. 3, the edge-side 18 conventionally comprises a network of stitches 22 made by means of an edge-to-side knit yarn 24, with an edge-side weft yarn 26 being interwoven between said stitches.
The leg portion 13 also typically comprises a mesh network 32 made by means of a leg portion knit yarn 34, a leg portion weft yarn 36 being interwoven between said stitches.
The nature of the threads and meshes is adapted according to the action desired for the different parts of the orthosis, in particular according to whether the action must be restrictive or compressive. This adaptation does not pose any particular difficulty to the skilled person.
For a CVE orthosis, the pressure measured at the ankle may in particular vary from 10 to more than 36 mmHg (ie 13 to 48 hPa, the mmHg being however commonly used as a unit of pressure measurement in the field of phlebology and of medical compression).
The CVE orthotics are distributed according to the ASQUAL standard in four textile classes, class I (13 to 20 hPa "10 to 15 mmHg at the ankle) to class IV (> 48 hPa"> 36 mmHg at the ankle).
A weft thread conventionally extends transversely to the main axis X of the orthosis, shown in FIG. 1. More precisely, it extends orthoradially, spirally around the main axis of the orthosis, passing along rows of stitches.
Score board
Preferably, the edge-side 18 is a so-called "double-reverse" structure, that is to say resulting from a folding of the neck of the orthosis (which defines the opening by which the orthosis is threaded) inward or, preferably, outwardly of the orthosis. It then comprises an inner lapel 18 ,, intended to be in contact with the skin of the lower limb of the patient, and an outer lapel 18e exposed to the outside and isolated from the skin by the inner cuff.
In a preferred embodiment, the edge-side knit yarn 24 is a single gimped yarn consisting of an elastane core A wrapped with one or more polyamide cover yarns C (see FIG. 4).
Preferably, the title (linear density) of the core of the edge-side knit yarn 24 is less than 45 dTex, preferably less than 30 dTex, preferably less than 25 dTex, preferably less than 20 dTex, and or greater than 10 dTex, preferably greater than 15 dTex.
Preferably, the title of the edge-side knit yarn cover yarn 24 is less than 60 dTex, preferably less than 50 dTex, preferably less than 45 dTex, and / or greater than 20 dTex, preferably greater than DTex, preferably greater than 40 dTex.
In one embodiment, the edge-dimension comprises only a single weft yarn.
In a preferred embodiment, the edge-side weft yarn 26 is a wrapped yarn, preferably double gimped.
The edge-side weft yarn 26 is made of an elastane core wrapped with elastane cover yarn, preferably double gimped with one or more elastane cover yarns. Double elastane wrapping facilitates the knitting of the orthosis.
Without being bound by this theory, the inventors consider that the elasticity of the cover wire prevents the cover wire from limiting the elastic deformation capabilities of the weft yarn.
Advantageously, an edge-to-side weft yarn 26 consisting of a double elastane core covered with an elastane cover yarn can be used as the conventional weft yarns, without a specific reeling system, unlike in particular single yarns. elastane.
Preferably, the title of the core of the edge-side weft yarn 26 is less than 150 dTex, preferably less than 140 dTex, preferably less than 135 dTex, and / or greater than 100 dTex, preferably greater than 100 dTex. 110 dTex, preferably greater than 120 dTex, preferably greater than 125 dTex.

In a preferred embodiment, the edge-dimension further comprises a reinforcing wire 40, different from the weft yarn and the knitting yarn, extending transversely, like the weft yarn.
In one embodiment, the edge-side has only a single reinforcing wire.
Preferably, the reinforcing thread does not extend over a part of the thigh and / or part of the ankle and / or part of the calf and / or part of the foot intended to wrap a thigh, ankle or calf and one foot, respectively, of a patient wearing the orthosis.
Preferably, the reinforcing thread extends exclusively in the edge-dimension, preferably over more than 90%, preferably substantially 100% of the height of the edge-dimension. Advantageously, the presence of the reinforcing thread in the single edge-side does not modify the behavior, and especially the elasticity, of the parts of the orthosis that extend under the edge-side.
Preferably, the reinforcing thread 40 extends only in the inner cuff 18 of the edge-side. The comfort is improved.
Preferably, the reinforcing thread 40 has an elongation at break of less than 40%, preferably less than 35%, preferably less than 30%, preferably less than 28%, and preferably less than 27%. The elongation at break can be measured according to DIN 53834.
The reinforcing thread 40 is preferably of polyamide. Preferably, the reinforcing thread 40 is a "simple" thread, that is to say not wrapped.
It preferably has a titre greater than 30 dTex, preferably greater than 35 dTex, preferably greater than 40 dTex and / or less than 60 dTex, preferably less than 50 dTex, more preferably less than 45 dTex.
The inventors have found that the presence of such a reinforcing thread substantially improves the crease resistance in the edge-side, without compromising comfort. Folds are areas of high pressure that affect the effectiveness of the orthosis.
Leg part
The leg portion knitting yarn may be the same or different depending on the portion of the leg portion considered. In particular, it may be different in the ankle 14 and calf 16 parts.
The knitting yarn may be identical in the calf and thigh portions 16 and 20 and / or in the ankle and thigh portions 20 and 20.
In one embodiment, the leg portion has only a single knit yarn.
In a preferred embodiment, the leg portion knit yarn 34 is a single gimped yarn consisting of an elastane core wrapped with one or more polyamide cover yarns.
